Cup Check-In Information

For teams that are involved in a play-in game:
  • Check-in will be held 1 hour prior to your scheduled game time.  There will be a MYSA representative on site to check you in.  
  • You will need to bring the following items to check-in:
    • Two (2) copies of your official, frozen State Cup roster.
    • Copies of the MYSA Medical Release forms for all players.  These will be verified on site by the MYSA representative and then returned to the teams.  The forms do NOT need to be notarized.  
    • Laminated Player & Coaches ID passes with a current photo.  Reprints will not be available on-site.  Passes DO NOT need to be signed anymore.

    • Each coach/team manager/bench personnel must bring a copy of their CDC Concussion test results showing they have passed.  CLICK HERE for instructions on taking the test.
  • Once you have checked-in for a play-in game, you DO NOT need to check-in again in St. Louis unless you change/add to your frozen roster.  

For teams that are NOT in a play-in game:
 
  • On Thursday night, May 29th, 2014 from 5:00 PM - 9:00 PM at the MYSA Office in St. Charles, MO
  • If you can't make the MYSA check-in, teams can check-in approximately 1 hour prior to their first game time at the MYSA administrative desk inside the SLYSA administrative trailer on site. There will be a MYSA representative there that will be able to check-in your team.
